Financial Arrangements for General Anesthesia in the Hospital
Fees for the physician time and facility usage will be partially or fully covered by your child's medical insurance. Our office will obtain authorization from your medical insurance prior to scheduling your child. You are responsible for all deductibles and/or co-payments associated with the hospital fees.
In addition to your portion of any hospital fees, you are responsible for the estimated patient portion for the dental treatment provided. This payment is due in full prior to the day of the surgery appointment. This patient portion is only an estimate and is subject to change pending radiographic and clinical examination under general anesthesia.
You are responsible for verifying medical and dental insurance coverage of benefits and eligibility prior to the surgery.
Payment may be made in the form of cash, Visa, Mastercard, CareCredit, or moneyorder. No personal checks will be accepted.
